Identify the Areas of Concern
- Identify the impact of the problem on the department or individual
- Confirm with all parties involved that the problem exists and needs to be addressed
- Agree on the intended result
Administer the appropriate Profile, Assessment or Survey
- Review results with supervisor and participant
- Prioritize the areas to be addressed
- Identify what the participant can do to correct the problem
- Generate as many alternative solutions as needed
- Evaluate the options
- Define the scope of training
Create an Action Plan
- Agree on the desired outcome or change
- Identify the resources and support needed
- Set the timelines for completion of each phase
- Establish measurable indicators of progress
Follow Up
- Review progress and address issues
- Determine whether the process is on schedule or completed
- Measure the success of the action plan
Implement Recognition / Rewards / Tracking Programs
- What were the results?
- What could have gone better?
- What exceeded expectations?
- What support has been put in place to insure success in the future?
